Access paradox: On medical education in India
The field of medical training in India has grown at such a breathtaking pace that it now prompts critical concerns regarding its availability. Over the past dozen years, availability has grown almost threefold, largely driven by state initiatives to enhance healthcare accessibility through better facilities and relaxed bureaucratic rules. Even so, the majority of this expansion occurs within private establishments, alongside newly established state colleges linked to regional hospitals. For the initial time this year, non-governmental facilities account for a majority of total MBBS availability. Among the approximately 10,000 newly introduced slots, 79 percent belong to these establishments, indicating that future expansion will likely follow this trend. This rise in availability is fundamentally positive, particularly because local youth continue to battle intensely for medical positions while medical facilities face a severe deficit of specialized doctors. Furthermore, countless learners pursue their studies overseas. Consequently, boosting local training capacity is a logical measure, and the growing availability should ease the current congestion while retaining skilled individuals within the nation.
Nevertheless, significant challenges persist. While studying for an MBBS in a state-run facility generally stays below ₹5 lakh for the entire course, attending a private facility can be ten times more expensive, driven partly by the constant need for medical services. The financial success of these private colleges tends to concentrate in specific regions, particularly major and secondary urban centers located in wealthier provinces where earnings are greater. Unless these newly qualified doctors intentionally relocate after completing their studies, their expertise remains gathered in these affluent hubs, whereas countryside locations severely lack adequate medical staff. This swift expansion also brings the danger of wildly inconsistent standards, as establishments often aim to pass National Medical Commission approvals using only minimal resources rather than thoughtfully tackling regional healthcare demands. Illegal donation fees, which continue despite official prohibitions, pile up fresh financial burdens that can shape professionals' career choices post-graduation. Lastly, lacking attractive wages, proper housing, educational facilities for children, and clear career growth, newly minted doctors frequently choose private clinics or move overseas. Ironically, this reliance on private expansion forces authorities to continuously scale up public infrastructure, such as establishing new AIIMS-style facilities, to ensure affordable options keep multiplying. The regulation requiring private colleges to offer half their slots at public rates must be strictly implemented, and the NMC should adopt strategies allowing unannounced visits and document reviews without prior authorization. Ultimately, rather than simply mandating rural postings, authorities ought to provide incentives for them, alongside guaranteeing that the everyday requirements of countryside medical staff, their households, and local clinics are adequately fulfilled.
